Insurance Accepted at Clear Sky Wellness, PLLC | HI, MN & ND
Clear Sky Wellness, PLLC works to make psychiatric mental health care and urgent care as accessible and straightforward as possible. We accept a growing number of insurance plans in Minnesota and North Dakota and also offer self-pay options for patients who prefer to pay directly for services.
Insurance participation can vary by provider, service, location, and plan. Coverage should always be verified before your first appointment. Being listed below does not guarantee that every service is covered or that Clear Sky Wellness, PLCC is considered in-network for every product offered by an insurance company.
Minnesota DHS Enrollment Delay
Clear Sky Wellness, PLLC is currently awaiting completion of its enrollment with Minnesota Health Care Programs (MHCP) through the Minnesota Department of Human Services (DHS).
Minnesota DHS continues to report a backlog in processing provider enrollment requests, including new enrollments, re-enrollments, and updates to enrollment information. DHS has stated that enrollment requests are currently being processed outside its standard 30-day processing timeframe.
Minnesota completed its statewide Revalidate 2026 provider revalidation determinations by May 31, 2026. DHS continues to address provider enrollment processing delays following this initiative.
Until Clear Sky Wellness, PLLC receives final approval from Minnesota DHS and completes any required health plan credentialing and contracting, we are unable to participate with or bill certain health plans, including Blue Cross Blue Shield of Minnesota, HealthPartners, and Minnesota Medicaid.
Hawaii Insurance Plans
Clear Sky Wellness provides eligible services to patients in Hawaii and may participate with select commercial, Medicare, or Medicaid insurance plans.

Minnesota Insurance Plans
Clear Sky Wellness, PLLC participates with select commercial insurance plans serving Minnesota.
Currently accepted Minnesota plans may include:
- America’s PPO
- Aetna
- Civilian Health and Medical Program of the Department of Veterans Affairs (CHAMPVA)
- Medica Minnesota
- United Behavioral Health (UBH) Commercial
- Sanford Health Plan
Some insurance companies offer multiple networks or products. Even if we participate with an insurance company, your specific plan may have different eligibility, referral, deductible, copay, coinsurance, or network requirements.

North Dakota Insurance Plans
Clear Sky Wellness, PLLC may provide services to eligible patients in North Dakota and participates with select North Dakota insurance plans.
Currently accepted North Dakota plans may include:

- United Behavioral Health (UBH) Commercial
- Sanford Health Plan
Additional insurance plans may be in the credentialing or contracting process. A plan will not be listed as accepted until Clear Sky Wellness, PLLC is authorized to bill that plan. Because coverage and network participation vary, please verify that your specific North Dakota insurance plan includes Clear Sky Wellness, PLLC and the services you are seeking.
Medicare
Clear Sky Wellness, PLLC accepts Medicare only when the treating provider and services are eligible for Medicare billing and current enrollment requirements have been completed.
Medicare Advantage plans are administered by private insurance companies and may have separate networks and authorization requirements. Participation with Original Medicare does not automatically mean participation with every Medicare Advantage plan.
Please contact us before scheduling if you plan to use Medicare or a Medicare Advantage plan.
Medicaid / Medical Assistance
Medicaid coverage differs between Minnesota and North Dakota and may also depend on the managed-care organization administering your benefits.

Clear Sky Wellness, PLLC continues to expand access to care by pursuing participation with additional insurance networks in Minnesota, North Dakota, and Hawaii.

Credentialing in progress does not guarantee future network participation or coverage. Clear Sky Wellness cannot bill a plan as an in-network provider until all required credentialing, contracting, and effective-date requirements have been completed.
If your insurance plan is listed as credentialing in progress, please contact Clear Sky Wellness before scheduling to discuss your current payment options. We can also let you know when participation has become effective.
Self-Pay
You do not need to use insurance to receive care at Clear Sky Wellness.
Self-pay may be a good option for patients who:
- Do not have insurance
- Have an out-of-network insurance plan
- Prefer not to submit services to insurance
- Have a high deductible
- Want a straightforward private-pay option
Additional services, testing, forms, or administrative requests may have separate fees when applicable.
Patients who are uninsured or choosing not to use insurance may be entitled to receive a Good Faith Estimate of expected charges under applicable federal law.
