First Responder & Military Mental Health Care
First responders, veterans, service members, and military families often face unique stressors that can affect mental health, sleep, relationships, work performance, and overall well-being.
Clear Sky Wellness provides individualized mental health and psychiatric care for people who may be experiencing the effects of critical incidents, trauma exposure, occupational stress, anxiety, depression, PTSD, sleep disruption, or other mental health concerns.
Johanna Christensen, DNP, APRN, FNP-BC, PMHNP-BC, EMT, is a Psychiatric-Mental Health Nurse Practitioner and Family Nurse Practitioner who is also a first responder and trained in Critical Incident Stress Management (CISM). Johanna Christensen has served as an EMT for more than 20 years. Her long-term experience in emergency medical services gives her firsthand insight into the demands, stress, shift work, critical incidents, and emotional impact that can accompany first responder careers.
First responder service is also deeply familiar to Johanna personally. Her father, husband, brother-in-law serve as firefighter-EMTs, her husband is also a MnFIRE instructor, and her brother-in-law works in law enforcement. These family connections give Johanna additional perspective on the demands first responders and their families may experience both on and off duty.
This background helps inform a respectful, practical approach to mental health care for first responders, veterans, service members, and their families.
Her background provides an additional understanding of the culture, demands, and stressors that can be part of first responder and military life.
Mental Health Care for First Responders
Law enforcement officers, firefighters, EMS professionals, dispatchers, corrections professionals, healthcare workers, and other first responders may experience repeated exposure to difficult, traumatic, or high-stress situations.
Sometimes the effects are immediate. In other situations, symptoms may develop gradually over time.
Mental health concerns may include:
- Difficulty sleeping
- Nightmares
- Anxiety
- Irritability
- Hypervigilance
- Depression
- Emotional numbness
- Difficulty concentrating
- Increased stress
- Changes in relationships
- Avoidance
- Feeling constantly “on alert”
- Trauma-related symptoms
- Post-traumatic stress disorder
- Burnout or emotional exhaustion
Seeking mental health care does not require waiting until symptoms become severe. Early support may help identify concerns and develop strategies before they begin to significantly interfere with daily life.
Mental Health Care for Veterans and Service Members
Military service can involve experiences and stressors that continue to affect a person long after deployment, transition, or separation from service.
Clear Sky Wellness provides psychiatric evaluation and treatment for veterans and service members experiencing mental health concerns such as:
- PTSD and trauma-related symptoms
- Anxiety
- Depression
- Sleep problems
- Irritability
- Mood changes
- Difficulty adjusting after deployment or separation
- Concentration concerns
- Occupational or relationship stress
- Medication-related concerns
Treatment is individualized based on symptoms, history, current health, personal goals, and clinical needs.
PTSD and Trauma-Related Care
Trauma can affect people in different ways.
Symptoms may include intrusive memories, nightmares, avoidance, irritability, increased startle response, sleep problems, emotional numbness, anxiety, depression, or feeling unsafe even when immediate danger has passed.
Care may include:
- Psychiatric evaluation
- Medication management
- Education about trauma-related symptoms
- Ongoing mental health follow-up
- Therapy recommendations
- Coordination with counselors or trauma-focused therapists
- Referral for additional services when appropriate
Treatment is based on the individual rather than assuming that everyone responds to trauma in the same way.
Psychiatric Medication Management
Medication can be helpful for some first responders, veterans, and service members experiencing PTSD, anxiety, depression, sleep problems, or other psychiatric symptoms.
Psychiatric medication management may include:
- Reviewing current symptoms
- Discussing medication options
- Evaluating benefits and risks
- Monitoring medication effectiveness
- Monitoring side effects
- Reviewing sleep, mood, anxiety, and daily functioning
- Adjusting medication when clinically appropriate
- Coordinating care with other healthcare professionals
The goal is to develop a treatment plan that supports functioning, safety, and quality of life.
Critical Incident Stress and Occupational Stress
Critical incidents can affect people differently depending on the event, previous experiences, support systems, workload, and individual circumstances.
Johanna’s training in Critical Incident Stress Management provides additional perspective when working with people who have experienced difficult events through emergency response, public safety, healthcare, military service, or similar professions.
CISM training does not replace individualized psychiatric care or psychotherapy. When appropriate, patients may also be encouraged to work with a therapist or other mental health professional experienced in trauma-focused treatment.
Sleep Problems
Sleep disturbance is common among people who work shifts, respond to emergencies, experience trauma, or remain in a heightened state of alertness.
Concerns may include:
- Difficulty falling asleep
- Frequent waking
- Nightmares
- Irregular sleep schedules
- Shift-work-related sleep disruption
- Difficulty relaxing after work
- Feeling tired despite sleeping
Sleep concerns may be evaluated as part of a broader mental health assessment because sleep can significantly affect mood, anxiety, concentration, physical health, and overall functioning.
Anxiety and Hypervigilance
Being alert to danger is an important part of many first responder and military roles. Sometimes that heightened awareness becomes difficult to turn off when the shift or mission ends.
Patients may experience:
- Constant worry
- Feeling on edge
- Muscle tension
- Racing thoughts
- Difficulty relaxing
- Increased startle response
- Difficulty sleeping
- Panic symptoms
- Feeling the need to constantly scan the environment
Evaluation can help determine whether symptoms may be related to anxiety, trauma, sleep disruption, occupational stress, medication effects, or another condition.
Depression and Emotional Exhaustion
Depression does not always look like sadness.
For some people, it may appear as:
- Irritability
- Loss of interest
- Low motivation
- Fatigue
- Isolation
- Difficulty concentrating
- Changes in sleep
- Emotional numbness
- Feeling disconnected from family or coworkers
Clear Sky Wellness provides individualized evaluation and treatment based on the person’s symptoms and goals.
Confidential, Respectful Care
Clear Sky Wellness understands that concerns about confidentiality, career impact, or stigma can make it difficult for first responders or military personnel to seek mental health care.
We strive to provide a respectful environment where patients can discuss concerns openly and participate in treatment decisions.
As with all healthcare services, confidentiality is maintained in accordance with applicable laws, professional standards, and required exceptions.
A Whole-Person Approach
Mental and physical health are closely connected.
Sleep, chronic stress, injuries, pain, medications, shift schedules, nutrition, relationships, and occupational demands can all affect mental health.
Johanna’s dual certification in psychiatric mental health and family practice allows her to consider both mental and physical health factors when appropriate.
Treatment may include psychiatric care, medication management, recommendations for therapy, medical evaluation, lifestyle strategies, or coordination with other healthcare professionals.
Telehealth Mental Health Care
Telehealth may be available for appropriate psychiatric evaluations, medication management, and follow-up visits.
Telehealth availability depends on:
- Clinical needs
- Patient location
- Provider licensure requirements
- The type of evaluation or treatment needed
Some concerns may require an in-person appointment or additional evaluation.
