Having an intrusive thought does not necessarily mean a person wants to act on it. However, these symptoms can be frightening and deserve compassionate, careful evaluation.
Patients should seek immediate emergency care if they believe they may harm themselves or another person, are unable to remain safe, or are experiencing severe confusion, hallucinations, or loss of contact with reality.
Postpartum Psychiatric Evaluation
A psychiatric evaluation may include discussion of:

- Pregnancy and delivery history
- Previous mental health concerns
- Current and past medications
- Sleep
- Feeding and breastfeeding considerations

- Thyroid or other relevant medical concerns

Medication Management During the Postpartum Period

- Hypervigilance
- Anxiety

Treatment may include psychiatric care, medication management, and referral for trauma-focused psychotherapy when appropriate.
When Symptoms Require Immediate Help
Some postpartum psychiatric symptoms require urgent or emergency evaluation.
Call 911, go to the nearest emergency department, or seek immediate professional assistance if there is:
- Immediate risk of self-harm or suicide
- Thoughts of harming the baby or another person with concern about acting on them
- Hallucinations
- Severe confusion
- Paranoia
- Extreme agitation
- Rapidly worsening behavior
- Loss of contact with reality
Postpartum psychosis is a psychiatric emergency and requires immediate evaluation.
