Clear Sky Wellness, PLLC welcomes referrals from primary care clinicians, therapists, counselors, psychiatrists, psychologists, naturopathic providers, chiropractic providers, hospitals, emergency departments, schools, community organizations, and other healthcare professionals.
We provide outpatient psychiatric evaluation, psychiatric medication management, mental health follow-up, and treatment for a range of behavioral health concerns through our Detroit Lakes and Hawley, Minnesota locations with telehealth appointments available when clinically appropriate and permitted by provider licensure requirements.
Our goal is to make the referral process straightforward while providing collaborative, patient-centered care.
Mental Health & Psychiatric Referrals
Providers may refer patients for evaluation and treatment of concerns including:
- ADHD
- Anxiety disorders
- Bipolar disorder
- Depression
- Diagnostic clarification
- Mood disorders
- Obsessive-compulsive disorder (OCD)
- Ongoing psychiatric follow-up
- Psychiatric medication evaluation

- PTSD and trauma-related conditions
- Sleep-related mental health concerns
- Stress-related conditions
Treatment recommendations are individualized based on the patient’s symptoms, history, medical considerations, current treatment, and clinical needs.
First Responder, Veteran & Military Referrals
Clear Sky Wellness, PLLC welcomes referrals for first responders, veterans, service members, and military families experiencing mental health concerns related to trauma, occupational stress, critical incidents, anxiety, depression, PTSD, sleep disruption, or other psychiatric symptoms.
Johanna Christensen, DNP, APRN, FNP-BC, PMHNP-BC, EMT has served as an EMT for more than 20 years and is trained in Critical Incident Stress Management (CISM), providing firsthand familiarity with first-responder culture and the challenges associated with emergency-service work.
Psychiatric Medication Consultation & Management
Referral may be appropriate when a patient needs:
- Initial psychiatric medication evaluation
- Review of an existing medication regimen
- Medication initiation when clinically appropriate
- Medication adjustments
- Monitoring for effectiveness or side effects
- Ongoing psychiatric medication management
- Additional evaluation when symptoms remain difficult to manage
Clear Sky Wellness, PLLC can collaborate with primary care, therapy, and other members of the patient’s healthcare team when appropriate and authorized.
What to Include With a Referral
Providing relevant clinical information can help us prepare for the patient’s evaluation.
When available, please include:
- Patient name and contact information
- Date of birth
- Insurance information
- Reason for referral
- Current symptoms or concerns
- Current medication list
- Previous psychiatric medications and response
- Relevant diagnoses
- Pertinent medical history
- Recent laboratory results when applicable
- Relevant psychiatric or psychological evaluations
- Hospitalization or emergency-care history when relevant
- Contact information for the referring provider

After a Referral Is Received
After receiving a referral, Clear Sky Wellness, PLLC will review the information and follow the practice’s scheduling and intake process.
Depending on the patient’s needs, insurance requirements, and provider availability, additional information may be requested before scheduling.
Patients may also be contacted directly to complete registration, insurance verification, consent forms, and other intake requirements.
Communication With Referring Providers
With appropriate patient authorization and when clinically indicated, Clear Sky Wellness, PLLC can communicate with referring clinicians regarding:
- Evaluation findings
- Treatment recommendations
- Medication changes
- Relevant laboratory or monitoring needs
- Follow-up recommendations
- Referrals to additional services
- Coordination of care
We value collaborative relationships with primary care providers, therapists, specialists, and other members of the patient’s healthcare team.
Insurance & Payment
Clear Sky Wellness accepts a range of insurance plans, including select Minnesota and North Dakota plans.

Telehealth Referrals
Telehealth may be available for appropriate psychiatric evaluations, medication-management appointments, and follow-up visits.
Eligibility depends on:
- Clinical appropriateness
- Patient location at the time of the appointment
- Provider licensure
- Applicable prescribing requirements
- Insurance coverage
Some concerns may require in-person assessment or additional medical evaluation.
